Many people with diabetes are hesitant about getting dental implants because they’re under the impression their chances of failure are greater than for non-diabetics. But if you’re one of the 26 million Americans with diabetes, that isn’t necessarily so — with a little extra precaution before, during and after implant surgery.
Diabetes is a group of diseases that affect how the body processes glucose. This simple sugar is used by the body to provide energy to cells, but can also cause damage if its volume level in the bloodstream is too high. The body normally regulates this through the hormone insulin produced by the pancreas.
The pancreas in people with Type 1 diabetes doesn’t produce insulin and so they must receive an outside source of the hormone through daily injections with careful daily monitoring of glucose levels. Those with Type 2 diabetes, the most common form of the disease, don’t produce a sufficient amount of insulin or the body no longer responds to the insulin produced. For either type, abnormal glucose levels — either too high or too low — can have adverse affects on the body, including blindness, nerve damage, gangrene, coma or death.
Diabetes can also slow wound healing, increase the risk of infection, and alter the body’s inflammatory response, all of which are major concerns when placing implants. Because implant placement involves minor surgery in which a wound results, there’s been wide concern that a slower healing process could increase the risk of implant failure.
Recent studies, though, are encouraging especially for patients who have their diabetes under control through medication, diet and exercise. Patients with poor glucose control are at higher risk, because it can take longer for the bone to heal around an implant after placement. For such individuals special considerations to guard against infection may be needed during implant surgery.
In fact, the implant success rate for most diabetics is about the same as for non-diabetic patients, 95%. With proper disease management and a little extra wound care, you can be among the many that experience a favorable outcome and a more attractive smile with dental implants.
National Fresh Breath Day is celebrated in August, but who doesn’t want fresh breath every day? Everyone has bad breath once in a while, so here are some tips to fight it.
1. Step up your oral hygiene routine.
Good oral hygiene is the first line of defense against bad breath. Brush your teeth morning and night and floss daily to remove much of the tiny food debris and plaque (colonies of oral bacteria) that can cause bad breath.
2. Don’t neglect your tongue.
A coated tongue can be a source of bad breath, so brush your tongue as well as your teeth or use a tongue scraper, which can be purchased in the oral health aisle of your local pharmacy.
3. Clean around your braces.
If you have braces, use an interdental brush or a water flosser to free trapped food particles.
4. Pay attention to your oral appliances.
If you wear dentures, be sure to clean them thoroughly every day, and brush your gums and the inside of your mouth as well. Bridgework also needs special attention: Clean carefully around the bridge and under the false tooth, as food can get stuck there.
5. Tackle dry mouth.
Dry mouth, a major cause of bad breath, can result from numerous medications, salivary gland problems, or breathing through the mouth instead of the nose due to sinus problems, sleep apnea, or other conditions. If your mouth is chronically dry, chew sugarless gum to stimulate saliva production, or ask about an over-the-counter or prescription saliva substitute.
6. Avoid extreme dieting.
Weight loss diets that advocate a stringent reduction in carbohydrates can lead to “keto breath.” This foul-smelling breath happens when the body burns fat instead of glucose for fuel.
7. Quit smoking.
In addition to smelling like cigarettes, people who smoke have less—as well as lower quality—saliva, which contributes to bad breath and poor oral health. If you need help quitting, talk with us or call (800) QUIT-NOW.
8. Be aware that some foods and beverages can leave stinky breath.
These include garlic, onions, strong spices, coffee, alcohol, cheese, and canned fish.
9. Keep up with regular dental visits.
Professional dental cleanings are necessary to get rid of hardened plaque (tartar) that can’t be removed by your brushing and flossing routine at home. We also check for gum disease, another cause of bad breath.
10. See your doctor.
Certain medical conditions like acid reflux, diabetes, and respiratory infections can cause bad breath. If you have an untreated health condition, make an appointment with your medical doctor.

You’re a bit self-conscious about your smile. But not because of your teeth — it’s your upper gums, which seem too prominent when you smile. While “too much” is a matter of perception varying from individual to individual, it’s generally accepted that a smile is “gummy” if four or more millimeters (a bit more than an eighth of an inch) of the gums are visible.
The good news is there are ways to improve the appearance of your gums. Which method we use, though, will depend on the underlying reason why the gums are prominent. The amount of gum tissue, in fact, may not be the problem at all, but could be the size of the crowns (the visible parts of teeth), the upper lip’s range of motion, the upper jaw’s position in relation to the face, or a combination of any of these.
For example, if your teeth didn’t erupt and develop properly, the gums might not have moved back to their proper position and stabilized as they should in your late teens or early twenties. A normal crown (the visible part of a tooth) is about 10 millimeters long, with a ratio of width to length of about 75-85%. Below those measurements the teeth can appear smaller, making even normal gum tissue appear larger. In another scenario, the upper lip may rise too high when you smile (hypermobility), which reveals too much of the gums.
If tooth size is the problem, we may recommend a periodontal surgical procedure called crown lengthening that reveals more of the tooth. A hypermobile lip can be treated with Botox shots to temporarily restrict the movement (it must be repeated every six months) or by surgically repositioning the lip muscles that control movement. Similarly, surgically repositioning an overlong upper jaw to make it appear shorter may be the right course.
